Using 3.2.1 on Fedora 19 with XFCE; on one monitor it's peachy.

Dock is on the bottom of my laptop screen. It hides when covered - great for limited vertical space.

When I plug in my external monitor, there's a problem.

The dock no longer pops back up. I surmise it is because the 'bottom' of the laptop screen doesn't meet the bottom of the virtual screen (2648 x 1024).

The 'always visible' icons show in the correct place.

Interestingly the 'Notification Area Old' gets placed in what appears to be a % equivalent place (was top right on the laptop screen, got moved to less-top-right of the external monitor).

On which monitor did you place the dock? (Advanced mode of the config panel / Position module). Do you have this problem if you place the dock on the left/right monitor (or on the middle).
Can you check if you're using XRanDR or Xinerama extension? => What's the output messages of these commands launched from a terminal:
xrandr
cairo-dock -l debug --colors | sed -n "/cairo-dock-X/ {N;p;}"

Using xrandr.

I haven't been at my home desk [where I use multiple monitors] since you asked, hence I haven't replied. I haven't forgotten!

Ok. But just to be sure that the dock has the right informations, what's the output messages produced by these commands:
xrandr
cairo-dock -l debug --colors | sed -n "/cairo-dock-X/ {N;p;}"
